Dysphagia in a patient with cervical diffuse idiopathic skeletal hyperostosis. Case report.

La hiperostosis esquelética difusa idiopática, es una patología caracterizada por osificación del ligamento longitudinal anterior. Aunque puede ser asintomática, su evolución clínica se vincula a síntomas compresivos de vías digestiva y/o respiratoria, que justifican una rehabilitación fonoaudiológica como tratamiento conservador inicial o postquirúrgico. Se reporta el caso de un hombre de 58 años atendido en consultorio clínico privado por presentar rigidez cervical y disfagia progresiva. La tomografía axial computarizada indicada reveló osificaciones cervicales (C3–C7) y dorsales altas (T1–T3) que comprimían el esófago. Tras el diagnóstico de disfagia mecánica extrínseca cervical, se instauró tratamiento médico y manejo de la sintomatología digestiva mediante terapias de rehabilitación fonoaudiológica.
Se realiza una discusión del caso con base a una revisión de la bibliografía científica relacionada al tema, con el objetivo de aportar información actualizada sobre esta enfermedad y los síntomas compresivos asociados, profundizando en el abordaje terapéutico implementado desde fonoaudiología para el manejo de la disfagia en este contexto.
Se detallan las estrategias de rehabilitación deglutoria aplicadas, incluyendo modificaciones posturales durante la deglución, adaptación de la consistencia del bolo según la clasificación de la International Dysphagia Diet Standardisation Initiative y ejercicios orientados a mejorar la coordinación respiración-deglución y su impacto en la evolución funcional del paciente. Estas estrategias permitieron una mejoría importante de los síntomas tras el primer mes de instauradas y sin necesidad de tratamiento quirúrgico a los siete meses de evolución, demostrando que la terapia conservadora es una opción no invasiva que puede resultar de manera favorable en esta enfermedad.
